Patient Forms
Patient forms can be used for new patient questionnaires (such as consent forms), surgery consent forms, or Patient Reported Outcome Measures (PROMS).
Create Patient Form templates in your Admin > Patient Forms area. For more information on this see Patient Form Templates
Create a Patient Form
Navigate to a Patient's Notes area, Click New and select Patient Form.
Click on "Show Patient Information Form" to populate the following patient details automatically:
- Title
- Firstname
- Middlename
- Lastname
- Known as
- Gender
- Date of Birth
- NHI
- Address
- Phone numbers
When these details are updated by the patient, they will auto-update the Elixir details saved for them.
Click on the "Show Consultant Signature Area" if the consultant needs to sign the form as well as the patient.
Click the INS button and choose which Patient Form template you wish to insert. The title will fill out automatically when you choose your patient form insert. Then hit create.
Form Options
Along the top you have a number of buttons.
- Get Link - Provides you with a link to open the form in another tab on your browser. This is useful when you want to open the form on an iPad for the patient to complete.
- Send to patient - Send it to the patient for filling in prior to coming in for their appointment.
- Open - Securely opens the patient form and logs you out of Elixir so that patients cannot navigate to other files.
- Edit - Edit the patient form.
- Delete - Delete the patient form.
- Print - Print the patient form.
Tip
Use your browsers Send Link to Device to easily send the form to your linked iPad etc if you want patients to fill out the form. You will need to be signed into the same browser account on your computer and iPad. Your IT Support provider can provide assistance on how to achieve this.
Patient Form Status
When a patient has filled in their form, it is sent to your Inbox > Patient Forms to be reviewed.
In Elixir Patient Forms have three different statuses:
- Unfilled (Red) - The patient is yet to fill in and submit the form to the clinic.
- Filled (Yellow) - The form has been submitted and is in the Inbox, ready to be reviewed.
- Reviewed (Green) - The form has been saved from the Inbox to the patient's file.
