Inserting a Template

To create a fillable PDF in a patient’s notes, click New and then select Fillable PDF.

1 - Fill out the relevant details, such as Consultant, Name, etc.

2 - The Referral field will automatically populate if the patient has only one referral.

3 - After selecting the template, the PDF fields will auto-populate based on the patient’s information. You can manually update or complete any fields as needed.

4 - Once complete, click Create.

You can also: Fillable PDF - Edit Mode

  • Edit the PDF by clicking the Edit button.

  • Download the PDF to your device.

  • Send the PDF directly to a contact.
