MoH Facility ID

Facility ID's

1. Check to see if you already have a HPI Facility ID at the Health NZ Website (Scroll down a bit to the Download - XLSX)

2. If you cannot find yourself in the spreadsheet, please email HI_Provider@health.govt.nz with the following information:

   • Name of Organisation
   • Name of the facility
   • Street address of the facility (and any other info such as building name if relevant)
   • Contact details for the facility – Phone, email
   • Type of facility – eg your specialty
   • Your name(s) and MC numbers.

3. They will email you back with your HPI Facility Id which can be loaded into Elixir under Admin > Team
