Patient Form Templates

Patient Forms have many uses. They can be used to create Patient consent forms or scoring forms such as Oxford knee score or SNOT22.

You can create patient form templates by going to the admin section of Elixir and navigating to Patient forms in the left panel. To create a New Template, click “New Template”

Give your patient form a name and enter into the content section what you wish to insert into your Patient Form.

You can use Elixir variables and Multi-select options within the Patient form templates.

When creating your templates, keep in mind that when you create a Patient form you have the option to “Show Basic Information Form” which includes the following:

• Title
• Firstname
• Middlename
• Lastname
• Known as
• Gender
• Date of Birth
• Email
• NHI
• Address
• Phone numbers

When these details are updated by the patient, they will auto-update the Elixir details saved for them.
