Patient Forms Guide

There are different ways that you can use Elixir's Patient Forms to gather information from your patients. Patients can complete forms prior to them coming into your clinic from home, or when they are present on clinic day.

Prerequisite

Have the forms that you are wanting to send created and ready to go. Have a look at our help page Patient Form Templates.

Using Patient Forms within in Clinic

Patients can complete electronic forms while they are in clinic by using a tablet or iPad. This could be used for Patient Consent forms or Surgery Consent forms for procedures taking place in a clinic.

  1. Make sure your tablet is connected to the internet.
  2. Go to a Patient's Notes page and create the forms you want them to fill in. Remember to use the Show Basic Information Form to automatically populate the patient's contact information.
    Show Consultant Signature Area can be enabled if the Consultant needs to also sign the form. Show Basic Information Form
  3. When the form is created, use the Open or Get Link option to open the form in another tab. Open your patient form
  4. You can now send that patient form to your tablet.

Sending Patient Forms Prior to Clinic Days

Sending a Patient form prior to a patient coming into the clinic allows you to have this information in advance. Send out forms such as Patient Consent Forms to gather the patient details or questionnaires on why they are coming to see you.

  1. Open a Patient's Notes page and create the forms you want them to fill in. Remember to use the Show Basic Information Form checkbox to automatically populate the patient's contact information.

  2. Send the Patient Form to the patient via email.

Hint

Create a task on that patient's file to check if they have completed the form. Complete the task when the completed form arrives in your Inbox.

Reviewing the Patient Form:

When the patient fills in the form it will come into your Inbox for you to review. Patient Forms Inbox If you used the Show Basic Information Form checkbox, anything the patient entered into those fields will automatically update in the patient file.

Other information entered in the forms may need to be manually transferred into areas of Elixir. For example if you have asked for ACC information, you may need update this in your Referral Custom Fields.

Save the Patient Form to the patient notes when you have finished reviewing it in the Inbox.

Sending Forms to a tablet

Chrome

  • Click on the Share botton on the screen (next to the URL/web address). Share button
  • Choose the option "Send to your devices" and find the name of your tablet/iPad
  • You will then be prompted on your tablet/iPad to open the form.

Firefox

  • Right click on the tab
  • Choose Send Tab to Device
